Submit duly accomplished TES 2 application form on or before August 15, 2019 (Thursday) at the Student Services and Development (SSD) Office, Room 205, FEM Hall (Administration Building) with the following documents:

1.  Certificate of Residency
2.  Certificate of Indigency/Low Income or Copy of Income Tax Return (if parent is government employee)
3.  Certificate of Registration (Form 5)
